The Palliative Care Case Manager will play a crucial role in guiding patients and their families through the healthcare system by assisting with access issues, coordinating resources, and providing disease-specific education. The ideal candidate will possess a deep understanding of both the clinical and emotional aspects of palliative care, and will act as a liaison between patients, caregivers, and healthcare providers. 

This role is a Part-Time opportunity and is field based in the San Francisco Bay area.

You will focus on:

  • Serve as the primary point of contact for patients and their families enrolled in the palliative care program.  

  • Provide emotional and logistical support for patients and their families to help them make informed decisions.  

  • See patients onsite when needed, to assess condition, review care plans, and provide immediate support.  

  • Facilitate timely communication between the healthcare team, patients, and their families.  

  • Coordinate healthcare resources and services to optimize patient care.  

  • Work collaboratively with an interdisciplinary team to develop care plans. 

  • Educate patients and caregivers on medication, symptom management, and available services.  

  • Provide information on advanced care planning and facilitate discussions if necessary.  

  • Work closely with external agencies and organizations to ensure continuity of care.  

  • Track and maintain detailed patient records, including consultations and interventions.  

  • Participate in team meetings, offering insights and feedback on patient cases. 

    The Skills, Experience & Education You Bring

    • Current LVN license in the state of California

    • Minimum of 2 years of clinical experience with experience in palliative or hospice care.

    The Base Salary for this position is $31.00 - $37.00/hr

Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.     The anticipated pay range for this role is listed in our salary posting for transparency but may vary based on factors including the candidate’s qualifications, skills, and experience. This position will also be eligible to participate in our annual bonus program.
