Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week
Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

What You’ll Do
• Manage a home health caseload across homes and community settings.
• Complete OASIS assessments and individualized care plans.
• Provide skilled nursing care and patient education.
• Coordinate care with physicians and interdisciplinary teams.
• Support patients in maintaining independence at home.
• Maintain accurate, timely documentation.
What We’re Looking For
• Active Minnesota Registered Nurse (RN) license.
• 3+ years of RN experience, home health preferred.
• Strong clinical judgment and independence.
• Organized and comfortable working in a field-based role.
• Reliable transportation.
