The LPN Supervisor fulfills a dual role, providing both direct operational support and supervisory oversight of the Population Health team. A significant portion of this role is dedicated to working at the top of their LPN license, including completing chart reviews for defined patient populations, performing outreach, and supporting key Population Health workflows.

Required Qualifications:
- Associate's Degree - Graduate of an accredited LVN/LPN school of nursing.
- Washington Licensed Practical Nurse upon hire.
- National Provider BLS - American Heart Association upon hire.
- 2 years - Experience in a medical clinic setting.
Preferred Qualifications:
- 2 years - Supervisory experience or Leadership in a medical Clinic or similar setting.\
- 1 year - Experience with Microsoft Excel.
- 1 year - Experience with Quality Improvement within healthcare field.
