About the Role:

Seniority Healthcare is seeking an experienced, tech-savvy Psychiatric Nurse Practitioner to join our growing clinical team in a fully remote capacity. In this role, you will provide psychiatric clinical support to seniors residing in senior living communities and private homes across New York and New Jersey through our expanding TelePsych service line.
This is an established clinical position — not a startup or greenfield role. You will step into an existing patient panel across our New York and New Jersey communities from day one, with immediate opportunities to expand your caseload as we grow our at-home psychiatric care program. All onboarding will be conducted remotely with full organizational support.
Key Responsibilities
- Deliver psychiatric evaluations, diagnosis, and ongoing medication management to seniors in senior living communities and private home settings via telehealth.
- Conduct comprehensive and problem-focused psychiatric assessments and develop individualized treatment plans.
- Order, review, and interpret diagnostic testing as clinically appropriate.
- Document all clinical encounters accurately and in a timely manner within designated electronic platforms and workflows.
- Coordinate care with community staff, internal clinical team members, patients, family members, and external providers to ensure continuity and quality of care.
- Support the development and expansion of Seniority Healthcare’s at-home psychiatric care model (PCM).
- Collaborate with behavioral health care managers, care coordinators, and other support roles as the program grows.
- Maintain compliance with all applicable New York and New Jersey regulations, licensure requirements, and organizational standards of care.
- Demonstrate self-sufficiency, strong clinical judgment, and professionalism in a fully remote care environment.
Required Qualifications
- Board Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
- Active Nurse Practitioner license in New York, in good standing.
- Active Nurse Practitioner license in New Jersey, in good standing.
- Demonstrated experience as a psychiatric clinician — candidates new to the field will not be considered.
- Strong technology skills and comfort with telehealth platforms, video conferencing, and electronic health records; tech proficiency is essential in this role.
- Excellent written and verbal communication skills; ability to articulate clinical findings and treatment plans clearly and professionally.
- Ability to work independently and self-sufficiently in a fully remote environment.
- Strong documentation, clinical assessment, and care coordination capabilities.
Preferred Qualifications
- Prior experience in senior-focused, geriatric, or community-based psychiatric care.
- Experience working within telehealth-only or hybrid care models.
- Familiarity with collaborative care models (CoCM), PCM, or evaluation and management workflows.
- Experience in senior living, assisted living, or memory care settings.
- Ability to work effectively alongside both clinical and non-clinical site partners remotely.
