Required: Active Washington Nurse Practitioner license + registered/enrolled with Washington Medicaid.
What You’ll Do
- Conduct Comprehensive Health Assessments through telehealth
- Review medical history, medications, and preventive care needs
- Document HCC (risk adjustment) findings accurately
- Close HEDIS (quality measure) care gaps
- Use ICD-10 and CPT II codes for documentation
- Deliver clear care plans and follow-up guidance
- Complete documentation on time
Must-Haves
- Active Nurse Practitioner license in Washington
- Registered or enrolled with Washington Medicaid
- 1+ year Family Medicine or Internal Medicine experience
- Experience caring for Medicare and Medicaid populations
- Familiarity with HEDIS and risk adjustment workflows
- Available at least 24 hours per week
Nice to Have
- Additional active state NP licenses
- Value-based care experience
- Annual Wellness Visit experience
- HCC risk adjustment documentation experience
