The Nurse Manager Discharge Care Management reports to the Nurse Director Discharge Care Management and is responsible for the leadership of a virtual team of Nurses.  We strive to provide our members with the most appropriate care, at the most appropriate time and in the most appropriate setting.

If you are located in EST or CST time zone, you will have the flexibility to work remotely* as you take on some tough challenges.

Primary Responsibilities: 

  • Oversees the Discharge care management area
  • Manage and oversight of a team size of approximately 20 nurses
  • Responsible for metrics of team and productivity reporting
  • Auditing of nurses to monitor that clinical criteria guidelines are being followed
  • Daily/weekly multidisciplinary case rounds
  • Required to create innovative approaches and solutions to meeting metrics
  • Participate in client facing meetings specific to performance guarantees required by the individual accounts
  • Participating in workgroups and initiatives
  • Support and drives change management for all initiative
  • Auditing and staff development, training, performance management and all hiring/ onboarding processes
  • Act as subject matter expert on projects
  • Participation in Regional/national committee meetings
  • Occasional local/national travel
  • Work queues management
  • Daily census management
  • Self-directed

Required Qualifications: 

  • Bachelor of Science degree in Nursing
  • Registered Nurse (RN), current and unrestricted license in State of Residence
  • 5+ years of clinical experience as a Registered Nurse
  • 5+ years of care management experience
  • 3+ years of Supervisory/leadership or Team Lead experience
  • 3+ years of clinical nursing experience practicing clinically (Acute Inpatient, SNF/AIR/LTAC, Emergency Department, Urgent Care, Doctor’s Office, School Nurse)
  • Experience in managed care
  • Discharge planning experience
  • Proven understanding utilization review process including census management
  • Proficient computer skills to include Microsoft Office (EXCEL. WORD, POWERPOINT), email, Internet and clinical programs
  • Proven excellent communication skills both verbal and written
  • Proven ability to manage client relationships in support of performance guarantees
  • Dedicated work space in the home and access to high speed internet

Preferred Qualifications:    

  • Experience working in a matrix organization
  • Knowledge of compliance and quality measures

Connecticut, New Jersey, New York, or Rhode Island Residents Only: The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
