Inova Fairfax Medical Campus is looking for a dedicated Registered Nurse Case Manager 2 Remote to join the team.

Registered Nurse Case Manager 2 Remote Responsibilities:
- The RN Case Manager 2 develops, implements and evaluates patient care plans and progression throughout the continuum of care or disease state.
- Works collaboratively in communication with physicians, nurses and other members of the multidisciplinary care team to effect timely and appropriate patient management.
- Provides discharge planning and continuity of care for assigned patients in acute and post-acute settings.
- Provides coordination of services and acts as key liaison between patients, families and interdisciplinary healthcare members.
- Uses utilization management techniques to determine the medical necessity, appropriateness and efficiency of the use of healthcare services, procedures and facilities.
Registered Nurse Case Manager 2 Remote Requirements:
Work Schedule: Monday – Friday 8:00am – 4:30pm with every 5th weekend and 1 holiday per calendar year.
Education: Bachelor’s Degree in Nursing or Associate’s Degree. ASN candidates must obtain BSN within 5 years of start date.
Experience: Minimum of two years experience in clinical care or clinical case management.
Licensure: Must be licensed as a Registered Nurse in the Commonwealth of Virginia or possess an active multi-state license.
Certification: Must be an accredited Case Manager, hold a Commission for Case Manager Certification or Care Guidelines Specialist Certification through MCG.
Additional Certification: Must hold a valid Basic Life Support certification through the American Heart Association.
Registered Nurse Case Manager 2 Preferred Requirements:
Current case management experience is highly preferred.
