Sentara Health is seeking to hire a qualified individual to join our team as a Team Coordinator, Care Management, Population Health.

 

Position Status: Full-time, Day Shift

 

Position Location: 3 days in office at our corporate office Sentara Park in Virginia Beach, VA – 2 days from home

 

Standard Working Hours: 8:00AM to 5:00PM

 

Minimum Requirements:

  • Required: Bachelors of Science in Nursing
  • Required: RN Licensure
  • Required: Case Management – 3 years

 

Position Overview:

Assumes responsibility, accountability and leadership for the daily operations including coordination of work, quality, and service. First line supervisor in the Department of Medical Care Management for assigned site/function. Facilitates the work of assigned team members. Provides a leadership role in ongoing case manager competency assessment, needs identification and educational offerings. Provides educational services to the Medical Care Management staff . Participates in the work activities of assigned teams and provides case management services as needed.

For Integrated Care Management departments, specialty certification required within one year of eligibility (ACM, CCM, CCCTM or RN-BC). For other service lines, certification based on specialty area required within one year of eligibility.

 

Position Responsibilities:

  • Reports directly to director of transitions
  • Responsible for daily assignments and staff coverage
  • Provides leadership with day-to-day supervision and coverage
  • Acts as a subject matter expert for transitions of care process and reports out on team performance
  • Provides training, ongoing competency assessments and educational opportunities
  • Meets at least monthly with individual team members
  • Meets at least monthly with other CM leaders, acute care leaders and other stakeholders
  • Leads team meetings and huddles
  • Participates in work activities of assigned teams as needed

Qualifications:

 

N-4YR – RN-Bachelor’s Level Degree (Required)

 

 

Basic Life Support (BLS) – Certification – American Heart Association (AHA) RQI, Registered Nurse (RN) Single State – Nursing License – North Carolina, Registered Nurse (RN) Single State – Nursing License – Virginia Department of Health Professionals (VADHP), Registered Nurse License (RN) – Nursing License – Compact/Multi-State License
